FREIGHT BRIDGE — INDEPENDENT CONTRACTOR / EQUIPMENT LEASE AGREEMENT (SUMMARY)

This Agreement is entered into between Freight Bridge ("Carrier") and the undersigned ("Driver/Contractor"). By signing, the parties agree to the following summary terms. The complete, legally binding agreement will be provided as a PDF for signature during onboarding.

1. Engagement. Carrier engages the Driver/Contractor to transport freight under Carrier's operating authority. Company Drivers operate Carrier-owned equipment; Owner-Operators lease their equipment to Carrier for the term of this Agreement.

2. Compensation. Settlements are paid weekly by direct deposit per the agreed gross percentage (30% Company Driver / up to 88% Owner-Operator), less applicable deductions disclosed in advance.

3. Equipment & Rentals. Trailer rentals, where applicable, are billed monthly at the rates published by Carrier. First-month trailer and insurance promotions apply to qualifying Owner-Operators.

4. Compliance. Driver/Contractor maintains a valid CDL, medical certification, and all FMCSA-required qualifications, and agrees to operate safely and lawfully at all times.

5. Term & Termination. Either party may terminate with written notice as set out in the full agreement. Outstanding balances are settled on the final settlement.

6. Authorization. Driver/Contractor authorizes Carrier to verify the information provided, including driving record, employment history, and banking details for settlement purposes.
